DO YOU ACCEPT INSURANCE?

Our office is private pay only, but we will provide you with the necessary information to file an out-of-network claim (for applicable commercial policies). You are responsible for obtaining all relevant insurance information, including referral requirements, coverage limits, exclusions, and prior authorizations. 

We recommend using Reimbursify to file out-of-network claims (https://reimbursify.com/). Prescription medication and some genetic testing may be covered by your insurance. 

We typically file prior authorizations (PAs) for prescribed medications as a courtesy and may ask you for a copy of your insurance card for this purpose.

 

WHAT AGES DO YOU SERVE?

We see patients from birth to adulthood. We do not provide services related to custody arrangements/disputes or dementia.

DO YOU REQUIRE A REFERRAL? 

Referrals are welcomed but not required. Patients can schedule appointments directly. 
Please note that some insurance companies require a referral to be considered for out-of-network benefits.

If you would like for your child (under 18) to be evaluated or treated…
We require parental consent from the biological parent(s) and/or legal guardian(s). 

If parents are married and living together, only one parent is typically required to give consent, though we reserve the right to require consent from both parents if deemed necessary. 

If parents are unmarried, separated, or divorced and have joint custody of the child, both parents must provide consent. 

We require that you provide all requested legal guardianship/custody documents.

HOW LONG DOES THE EVALUATION PROCESS TAKE?

Some evaluations are completed in one visit. 
Comprehensive evaluations typically take several visits, including an initial visit, several testing visits, and a feedback visit.
